India has reduced new infections and expanded access to ART through evolving strategies across NACP phases.<\/strong><\/span><\/li>\n<\/ul>\n<\/div>\n<p style=\"text-align: justify;\"><span style=\"color: #3366ff;\"><strong>Introduction<\/strong><\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"color: #3366ff;\"><strong>World AIDS Day is a global observance held on December 1 every year to raise awareness about the HIV\/AIDS epidemic, remember those who have died from HIV-related illnesses, and support people living with HIV\/AIDS. It was first marked in 1988 by the World Health Organization (WHO) and has since become a platform for governments, communities, and individuals to unite in the fight against the disease. The theme for this year is\u00a0\u201cOvercoming disruption, transforming the AIDS response.\u201d\u00a0It emphasises the need to not only preserve past progress but transform HIV services to make them more resilient, equitable, and community-led. This theme highlights the urgency of addressing disruptions caused by pandemics, conflicts, and inequalities that limit access to care. India marks World AIDS Day each year through nationwide awareness campaigns, community outreach activities, and renewed government commitments led by the National AIDS Control Organization (NACO) under\u00a0the Ministry of Health &amp; Family Welfare.<\/strong><\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"color: #3366ff;\"><strong><img decoding=\"async\" src=\"https:\/\/static.pib.gov.in\/WriteReadData\/userfiles\/image\/image003YUTG.jpg\" \/><img decoding=\"async\" src=\"https:\/\/static.pib.gov.in\/WriteReadData\/userfiles\/image\/image004ZPXB.png\" \/><img decoding=\"async\" src=\"https:\/\/static.pib.gov.in\/WriteReadData\/userfiles\/image\/image0051FTO.jpg\" \/><\/strong><\/span><\/p>\n<p style=\"text-align: justify;\">\n<p style=\"text-align: justify;\"><span style=\"color: #3366ff;\"><strong>India\u2019s Journey<\/strong><\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"color: #3366ff;\"><strong><img decoding=\"async\" src=\"https:\/\/static.pib.gov.in\/WriteReadData\/userfiles\/image\/image006D677.png\" alt=\"\" \/><\/strong><\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"color: #3366ff;\"><strong>India\u2019s AIDS Control Programme is globally acclaimed as a success story.<a style=\"color: #3366ff;\" title=\"\" href=\"https:\/\/www.pib.gov.in\/PressReleasePage.aspx?PRID=2196475&amp;reg=3&amp;lang=1#_ftn1\" name=\"_ftnref1\">[1]<\/a>\u00a0The initial phase (1985-1991) focused on identifying HIV cases, ensuring safe blood transfusions, and generating targeted awareness. The response gained momentum with the launch of the National AIDS and STD Control Programme (NACP) and the National AIDS Control Organisation (NACO) which was established in 1992 under the Ministry of Health and Family Welfare to coordinate a multi-sectoral national strategy.\u00a0Over time, the focus of NACP shifted from a national response to a more decentralised response and to increase involvement of NGOs and networks of People Living with HIV (PLHIV).<\/strong><\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"color: #3366ff;\"><strong><img decoding=\"async\" src=\"https:\/\/static.pib.gov.in\/WriteReadData\/userfiles\/image\/image0124SOX.png\" alt=\"\" \/><\/strong><\/span><br \/>\n<span style=\"color: #3366ff;\"><strong><img decoding=\"async\" src=\"https:\/\/static.pib.gov.in\/WriteReadData\/userfiles\/image\/image013CZNP.png\" \/><img decoding=\"async\" src=\"https:\/\/static.pib.gov.in\/WriteReadData\/userfiles\/image\/image014C9GJ.png\" \/><\/strong><\/span><\/p>\n<p style=\"text-align: justify;\">\n<p style=\"text-align: justify;\"><span style=\"color: #3366ff;\"><strong>The National AIDS Control Programme (NACP)<\/strong><\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"color: #3366ff;\"><strong>It has evolved through five phases, shifting from basic awareness to comprehensive prevention, testing, treatment, and sustainability.<\/strong><\/span><\/p>\n<h3 style=\"text-align: justify;\"><span style=\"color: #3366ff;\"><strong>NACP I (1992\u20131999)<\/strong><\/span><\/h3>\n<ul style=\"text-align: justify;\">\n<li><span style=\"color: #3366ff;\"><strong>Launched India\u2019s first comprehensive HIV\/AIDS prevention and control programme.<\/strong><\/span><\/li>\n<li><span style=\"color: #3366ff;\"><strong>Aim:\u00a0Slow the spread of HIV\u00a0and reduce\u00a0morbidity, mortality, and overall impact\u00a0of AIDS.<\/strong><\/span><\/li>\n<\/ul>\n<h3 style=\"text-align: justify;\"><span style=\"color: #3366ff;\"><strong>NACP II (1999\u20132006)<\/strong><\/span><\/h3>\n<ul style=\"text-align: justify;\">\n<li><span style=\"color: #3366ff;\"><strong>Focused on two key objectives:<\/strong><\/span>\n<ul>\n<li><span style=\"color: #3366ff;\"><strong>Reduce the spread of HIV\u00a0in India.<\/strong><\/span><\/li>\n<li><span style=\"color: #3366ff;\"><strong>Strengthen long-term national capacity\u00a0to respond to HIV\/AIDS.<\/strong><\/span><\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<h3 style=\"text-align: justify;\"><span style=\"color: #3366ff;\"><strong>NACP III (2007\u20132012)<\/strong><\/span><\/h3>\n<ul style=\"text-align: justify;\">\n<li><span style=\"color: #3366ff;\"><strong>Goal:\u00a0Halt and reverse the HIV epidemic\u00a0by 2012.<\/strong><\/span><\/li>\n<li><span style=\"color: #3366ff;\"><strong>Strategy:<\/strong><\/span>\n<ul>\n<li><span style=\"color: #3366ff;\"><strong>Scale up prevention among\u00a0High-Risk Groups (HRGs)\u00a0and the\u00a0general population.<\/strong><\/span><\/li>\n<li><span style=\"color: #3366ff;\"><strong>Integrate\u00a0prevention, care, support, and treatment\u00a0services.<\/strong><\/span><\/li>\n<\/ul>\n<\/li>\n<li><span style=\"color: #3366ff;\"><strong>Key addition: Creation of\u00a0District AIDS Prevention and Control Units (DAPCUs)\u00a0for district-level coordination and monitoring, including stigma\/discrimination reporting.<\/strong><\/span><\/li>\n<\/ul>\n<h3 style=\"text-align: justify;\"><span style=\"color: #3366ff;\"><strong>NACP IV (2012\u20132017)<\/strong><\/span><\/h3>\n<ul style=\"text-align: justify;\">\n<li><span style=\"color: #3366ff;\"><strong>Goal:\u00a0Accelerate reversal\u00a0of the epidemic and ensure an\u00a0integrated response.<\/strong><\/span><\/li>\n<li><span style=\"color: #3366ff;\"><strong>Objectives:<\/strong><\/span>\n<ul>\n<li><span style=\"color: #3366ff;\"><strong>50% reduction in new infections\u00a0(compared to 2007 baseline).<\/strong><\/span><\/li>\n<li><span style=\"color: #3366ff;\"><strong>Provide\u00a0comprehensive care, support, and treatment\u00a0for all PLHIV.<\/strong><\/span><\/li>\n<\/ul>\n<\/li>\n<li><span style=\"color: #3366ff;\"><strong>Extended (2017\u20132021)\u00a0to advance the goal of\u00a0Ending AIDS by 2030.<\/strong><\/span><\/li>\n<li><span style=\"color: #3366ff;\"><strong>Major initiatives during extension:<\/strong><\/span>\n<ul>\n<li><span style=\"color: #3366ff;\"><strong>HIV\/AIDS (Prevention and Control) Act, 2017)-\u00a0It prohibits discrimination against people living with HIV (PLHIV), ensures confidentiality, and mandates informed consent for testing and treatment while promoting prevention and access to care.<\/strong><\/span><\/li>\n<li><span style=\"color: #3366ff;\"><strong>Mission Sampark-\u00a0Its purpose was to \u201cbring back\u201d people living with HIV (PLHIV) who had stopped antiretroviral therapy (ART) \u2014 i.e. to trace and re-engage those \u201clost to follow-up\u201d.\u00a0It uses a community-based testing and follow-up approach.<\/strong><\/span><\/li>\n<li><span style=\"color: #3366ff;\"><strong>\u2018Test and Treat\u2019 policy\u00a0(initiating ART for all diagnosed cases)<\/strong><\/span><\/li>\n<li><span style=\"color: #3366ff;\"><strong>Routine Universal Viral Load monitoring<\/strong><\/span><\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<h3 style=\"text-align: justify;\"><span style=\"color: #3366ff;\"><strong>NACP V (2021\u20132026)<\/strong><\/span><\/h3>\n<p style=\"text-align: justify;\"><span style=\"color: #3366ff;\"><strong>Launched as a Central Sector Scheme with an outlay of Rs. 15,471.94 crore, Phase-V aims to build on past achievements and address persistent challenges. Mass media, digital platforms, and social media are used to reach broader and younger audiences.<\/strong><\/span><\/p>\n<ol style=\"text-align: justify;\" start=\"2\">\n<li><span style=\"color: #3366ff;\"><strong>Expanded Outdoor Outreach<\/strong><\/span><\/li>\n<\/ol>\n<p style=\"text-align: justify;\"><span style=\"color: #3366ff;\"><strong><img decoding=\"async\" src=\"https:\/\/static.pib.gov.in\/WriteReadData\/userfiles\/image\/image019BUV8.png\" alt=\"\" \/><\/strong><\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"color: #3366ff;\"><strong>Awareness strengthened through hoardings, bus panels, information kiosks, folk performances, and IEC vans. These tools help promote services and facilities available across the country.<\/strong><\/span><\/p>\n<p style=\"text-align: justify;\">\n<ol style=\"text-align: justify;\" start=\"3\">\n<li><span style=\"color: #3366ff;\"><strong>Community-Level Awareness Programs<\/strong><\/span><\/li>\n<\/ol>\n<ul style=\"text-align: justify;\">\n<li><span style=\"color: #3366ff;\"><strong>Training and sensitisation conducted for Self-Help Groups (SHGs), Anganwadi Workers, ASHAs, Panchayati Raj members, and others. These grassroot initiatives promote behavioural change and overall community awareness.<\/strong><\/span><\/li>\n<\/ul>\n<ol style=\"text-align: justify;\" start=\"4\">\n<li><span style=\"color: #3366ff;\"><strong>Targeted Interventions for High-Risk Groups<\/strong><\/span><\/li>\n<\/ol>\n<ul style=\"text-align: justify;\">\n<li><span style=\"color: #3366ff;\"><strong>1587 Targeted Intervention projects rolled out across the country as on October \u00a02025. India&#8217;s AIDS decline is more prominent than the global average, supported significantly by broadened testing, enhanced access to antiretroviral therapy, focused outreach to high-risk groups, and initiatives to combat stigma, all implemented through collaborative state and community actions.
